This is a sheet I have used with Youth Leaders to help them access how they are going, It can be a good starting point for helping people to see where they need to improve.
Time
 1  Normally very late or don't turn up.
 2. Normally late and regularly don't turn up.
 3. Occasionally late.
 4. Get there just after the group begins.
 5. Get to the group a little early.
 6. Arrive early enough to be with the leaders planning.

Commitment and reliability.
 1. Hardly ever turn up or warn people.
 2. I regularly don't let people know I am not coming.
 3. Regularly miss the group but warn the other leaders but normally at the last moment.
 4. Occasionally miss the group but for no real reason.
 5. If I ever miss I check with the key leader to make sure it is okay.
 6. Only miss the group if I can find a replacement for the night and the reason is very important.

Preparation
 1. Look at the study at the group for the first time.
 2. Read the study during the group.
 3. Read the study before the group.
 4. Prepare the study before I arrive at the group.
 5. I have prepared the studies for the next couple of week.
 6. I have prepared all the studies for the year.

Relationship with the members
 1. I rude to the members.
 2. Never speak to them.
 3. Speak as little as possible with them.
 4. Only talk to them in the small groups.
 5. Talk regularly with them.
 6. Contact them during the week or at Church etc.

Relationship with the other leaders
 1. Treat them like dirt.
 2. Act unhelpful to them.
 3. Go against them during the group.
 4. Act neutral to them.
 5. Seek them out and work well with them.
 6. Work well and pray regularly for them.

Discipline
 1. Let the members get away with anything.
 2. Sometimes tell the members not to do the wrong thing.
 3. Maintain unclear standards of Discipline.
 4. Try to keep the group under control.
 5. Have clear standards and the members respect my position.
 6. Thoughtfully discipline and support other leaders when there discipline.

Training
 1. Never bother learning anything.
 2. Will do any training only if I have to.
 3. Continually put it off.
 4. Do the minimum I have to.
 5. Do training courses regularly.
 6. Do training courses and read books regularly on leadership.

Godliness
 1. My example and spiritual life is dead.
 2. My example and spiritual life is poor.
 3. My example and spiritual life is not regular.
 4. My example and spiritual life is improving.
 5. My example and spiritual life is something that points to Christ.
 6. My example and spiritual life honours Christ.
